Health Coverage Tax Document Information for DCG Staff Members

Dallas Center-Grimes Community School District is no longer required to automatically mail printed copies of the 2025 Forms 1095-B and/or 1095-C to individuals. These forms provide information about the health coverage offered to you and your family, as required by the Affordable Care Act (ACA). The information on these forms has been filed with the IRS. 

How to Request Your Form:

If you would like to request a paper copy of your 2025 Form 1095-B or 1095-C, you may do so by contacting us using one of the methods below:

• Forms will be provided to the requester by the later of January 31, 2026, or within 30 days of receiving your request.
